If you have a working headset for your PS3 please post! You will be helping all of us out.
Here's a response format:
* Mic Brand
* Model #
* Mic needs to be initialized after every re-start?
* Include anything else we might need to know.
* If you have any information about how the chat works in specific games please post!
The Bluetooth Headset Guide
The PS3 uses Bluetooth ver. 2.0, and is compatible with all earlier versions. The PS3 can support up to 7 Bluetooth devices at once, including controllers.

Have the instructions for your headset at hand when you pair it with your PS3. Some headsets have special instructions specific to that model.
If your still having trouble, here's a picture guide with step by step instructions by kman79

Here's a post from this thread; a verbal step-by-step on how to set up your bluetooth headset by *(SONY4LIFE)*. Look for post #404:

USB Headsets
Any wired USB headset that works with PS2 will be fine for PS3 games. PS2 games are not compatible with Bluetooth, so you will need a USB headset for any PS2 games.
Just plug it into the front USB port and go to the "Settings" menu in the XMB. Choose "Accessory Settings", then "Audio Device Settings" and choose your mic for Input and Output. You can also adjust the mic level in the same section.
General Info:
*A note on Firmware Update 1.60
* It appears that some of the Bluetooth headset pairing issues have been resolved.
* Go back and try that headset that had problems before! You might be pleasantly surprised.
* So far, the Motorola HS850 stays paired as confirmed by some users. I will update the list below as information comes in.
*A note on pairing problems:
* Some headsets have a "pairing mode" that you need to turn on before you can pair it with any device, so check the headset instructions.
* To turn on pairing mode on most headsets: hold the call button for about five seconds until the status light changes. Then have the PS3 scan for your headset. Be careful because it's easy to enter "pairing mode" by accident when turning on a mic, and you'll have to re-sync if you do.
* If you need a "passcode" try entering 0000. All the mics seem to use 0000 as the default.
* If you use the same headset for your phone and PS3, you may have to re-pair for each play session. Also, after charging your headset you may have to re-pair.
*A note on voice chat drops:
* Some users report mics cutting out in game. If the problem is not game related running the setup again can help. Be sure to have the headset instructions handy. System updates, and game patches, might be affecting the headsets- need more on that one though.
*A note about controller interference:
* If your controller loses contact when you turn on your BT mic, they are probably interfering with each other. You can try changing the controller port assignments in the XMB:
1. During a game hold the PS button and the settings screen will come up. You can now change the controller port assignment.
2. Some games might need the controller assigned to a specific port. The game instructions will tell you.
3. Each time you turn on the PS3 you will need to set the port assignment.

*A note on stereo mics:
* If you have a stereo headset mic (2 speakers) it will only play the voices of the other players. Game sounds and music will still play over your regular speakers.
*A note on PS2 USB mics:
* PS2 USB mics are working with PS3 games.
Game Specific Info:
Resistance: Fall of Man
* There is a new update (3/20/07) that may address mic problems. They have fixed a bug that made the voice chat degrade during a long game. Check the R:FoM board for the full details.
* A second update seems to have affected the voice chat in-game. There are a few more reports of mics cutting out. Also some report that their mic works for one game, but then fails in RFOM. Maybe only some brands? Need more info here.
* The first patch has changed the voice function. Now press L3 to talk in lobby and in game. If you used the "Form Party" option, the mic is open to those in your Party. You must press L3 to talk to everyone else.
* There have been many reports of the mics dropping out in-game. Odds are that this is just a typical online game problem, not a hardware issue. If anyone notices anything else about this, please post.
Armored Core 4- There is no voice chat, only "canned" comments.
Blazing Angels- There is no voice chat.
Call of Duty 3- There is no voice chat.
Calling All Cars
College Hoops 2K7 (PS3)
F.E.A.R.
Fight Night Round 3 (PS3)- Uses an open mic, just talk no button press.
Formula 1: CE
Full Auto 2: Battlelines- There is no voice chat or text chat support.
Madden NFL '07-
* Voice chat works in-game only. There is no voice chat in the pre/post game lobbies.
* There is a "mute" key built in the controls. L3 will mute your headset. If the screen icon is red then your mute is on and you can't talk. If your icon is green then you're good to go.
Major League Baseball 2K7 (PS3)
MLB 07
MotorStorm-
* Mics are "open" meaning you don't have to press a button, just talk.
* Seems like only 2 people can talk at once, so be patient.
* If the server detects lag/latency the mics are suspended temporarily (your icon should turn red?) to keep the game running smooth.
Need For Speed: Carbon
NBA 2K7
NBA Street Homecourt
Rainbow Six: Vegas
Ridge Racer 7- Only supports "canned" comments- think macros, or like in Resident Evil: Outbreak.
Rogue Warrior
SOCOM (PS2)- SOCOM games will not work with your Bluetooth headset. You can still use a USB mic for SOCOM games.
Splinter Cell: Double Agent (PS3)
Tiger Woods 2007 (PS3)- Voice chat works best if you have your mic on before starting the game.
Headset Brand and Model Listing: (brand names are linked)
The numbers listed here are taken from posts in this thread and by actual users. The numbers are a guide to help show trends, they are not a popularity contest or any kind of endorsement. I am counting "paired at restart" as a problem because it is in my opinion. Not everyone may agree, so if yours works for you, great!
After Firmware Update 1.60: Many of the pairing problems have been resolved! As I get more information I will update the list to show what is working.
Motorola (Model H700 has an "A" and "B" version with different pairing procedures. Here's the link to Motorola FAQ to help with the issue)
ALERT: Beware of FAKE Motorola BT headsets! The fakes have a lot of problems. Think yours is counterfeit? Check the .pdf (thanks Amazon.com): Link: Motorola Counterfeit I.D. Guidebook
