What PSP firmware version do I need?

You'll need the OE firmwares from Dark_Alex.
At least 3.03 OE-C!
IT DOES NOT WORK WITH THE OFFICIAL 3.03 FW PROVIDED BY SONY!
Attempting to run the generated Eboots prior to
3.03 OE-C will fail!

Check your psp firmware version if it is 3.03 OE-C.
Some PSX games make use of subchannel data.
In order to retrieve them and integrate them into the eboot
dump your PSX cd with CloneCD or Alcohol (as CloneCD Image)
and convert it again
However it could be also down to IceTea in this case wait for
an update.

Using IceTea
------------
Simply execute IceTea, everything is well explained so you
should't have a hard time figuring out the workflow
When you've converted an image, plug your psp usb cable in and make
a new directory in /PSP/GAME/ eg. "ResidenEvil"
Then copy over your newly built eboot to /PSP/GAME/ResidentEvil/
The manual must also be placed in the same directory as the Eboot and
named "document.dat" otherwise the psp is not able to detect it!
