After 5 Days of research (10 Hours) we now have the details on the fix.
This new fix is now know as the Chez fix named after the Computer Engineering team that developed it. Please make this a sticky. This is an advanced fix. You must be knowledgeable in electronics.
STEP ONE: Follow this ideas in the Sticky first (at the top of the posts). Most of the time this is all you will need to fix the issue.
STEP TWO: If that does not work:
Check all the traces using a meter using this diagram. This was posted in an other Thread.
I big thanks to the poster. This is often the cause for bad soldered pins.

STEP THREE:
Try a differnt Mod Chip. We have found some to come with bad BIOS Software installs. ReFlash or try a different one.
STEP FOUR: If that does not work, then you need to do a Diode/resistance check.
This is often the cause from either bad boards from the factory or from damage to the board from you over heating or droping solder in the wrong place.
To do this you need a Diode checker that will also give you readings on the Diodes.
Radio Shacks part# 22-810 will work. You are not really checking Diodes. What you are checking is the basic circuit flow of the board to the LPC port.
Using the diagram from the link above (looking at the bottom of the m board)
the reading should be:
11) Black Ground wire 1) 1.4
for all tests
0) no pin 2) 1.4
10) .3 3 ) 1.4
9) 1.4 4) 1.4
8) 1.4 5) .3
7) 0 6) 1.4
0) no pin 0) no pin
0) no pin 0) no pin
All readings are Plus/minus .2
In most cases you will find that Pin #4, 10 and #5 will be the ones with the issues.
Pin #5 is most likely the problem. As you will note there are no leads running to it from the top or the bottom. It has a lead in the middle of the main board that leads to the trace as documented in the web link above. You may need to pull you pin in and out till this contact read correctly. The contact is in the middle of the board.
Pin #4 and 10 are often caused by bad resistors or resistors that you have messed up from Solder falling on them or resistors that are now bad from over heating the board. Check all resistors around the pins. Most likely it is the resistors above PIN #1.
We have fixed over 4 X-boxes with Executor II install issues using this method.
