Please go to the SETTINGS page within your Lockerz account (login and click on SETTINGS in the upper right of your Lockerz page). Then please provide any information that is missing in your SETTINGS and please correct any information in your SETTINGS that is incorrect. Be sure to use English letters. If you live outside the US, please provide your phone number (this is required by UPS for shipping outside the US).
REMEMBER, PLEASE CHANGE YOUR INFORMATION IN "SETTINGS." DO NOT EMAIL YOUR INFORMATION TO LOCKERZ.
Please provide this information by Tuesday, December 22, 2009. If you have not provided the information we need by this time, we will be unable to send you your prize.
